Job Description
We are seeking a skilled and compassionate Nurse Practitioner (NP)/ Physician Assistant (PA) or Medical Doctor (MD/DO) to support facilities in Merced, California. This role focuses on providing high-quality care to patients in skilled nursing and assisted living facilities, primarily within post-acute and long-term care settings.

Position Details
• Part-time, per diem role (approximately 2–4 shifts per month)
• Hybrid model (in-person care + remote patient monitoring)
• Flexible scheduling based on clinical needs

Key Responsibilities
• Conduct comprehensive patient assessments and physical examinations
• Develop and manage treatment plans in collaboration with supervising physicians
• Review and interpret laboratory results and diagnostic data
• Provide patient and family education on health management and chronic conditions
• Document patient care accurately in electronic medical records (EMR)
• Coordinate care with interdisciplinary teams and facility staff
• Support ongoing patient monitoring and follow-up care

Qualifications
• Active and unrestricted NP or PA license in California
• Experience in post-acute, primary care, or related clinical settings preferred; open to candidates with strong clinical training and interest in learning
• Strong clinical judgment, communication, and interpersonal skills
• Experience with EMR systems and comfort with telehealth or remote patient monitoring (preferred)
• Ability to work independently while collaborating effectively with a care team
