What You’ll Actually Do:

• Provide face-to-face care in patient homes and facilities
• Make real-time clinical assessments that guide hospice eligibility and palliative decisions
• Work directly with hospice physicians and attending providers to shape care plans
• Educate families during some of the most important conversations of their lives
• Support and elevate the interdisciplinary team with your clinical expertise
• Document and communicate findings that directly impact patient comfort and care decisions

Requirements

• Current STATE RN and Advanced Practice Nurse License

• Valid Driver’s License

• Valid Auto Insurance

• AHA CPR Certification

• HomeCare HomeBase experience preferred
