Job Title: Primary Care Nurse Practitioner or Physician Assistant
Job Type: Full-Time
Schedule: Monday–Friday with half Admin Day mid week as volume builds up.

Responsibilities:
- Deliver high-quality primary care, including health assessments, diagnosis, and management of acute and chronic conditions
- Focus on preventive health, wellness education, and lifestyle counseling
- Collaborate closely with fellow Nurse Practitioners and support staff to ensure coordinated, patient-focused care
- Maintain accurate and timely clinical documentation in the electronic medical record (EMR)
- Participate in practice improvement efforts and team meetings
Qualifications:
- Current Nurse Practitioner/PA license and board certification (FNP, ANP, or equivalent)
- Minimum 3 years of experience in a primary care setting
- Strong clinical judgment and commitment to holistic, evidence-based care
- Excellent interpersonal and communication skills
- Ability to work independently while collaborating effectively with a team
- Knowledge and experience with EclinicalWorks (eCW) EMR is highly desired
