Position Overview

We are seeking a Nurse Practitioner (NP) or Physician Assistant (PA) to provide part-time coverage during nights, weekends, or both.

What You’ll Do

Provide on-call and urgent care support for ALF/SNF patients

Manage acute changes in condition via:
- Telehealth
- Phone triage
- Facility collaboration

Coordinate with facility staff and nursing teams

Document encounters in our EHR (Ethizo)

Ensure high-quality, compliant clinical decision-making
