Primary Responsibilities and Duties
- Provides diagnosis and treatment to manage patient health conditions.
- Performs physical exams at initial and follow-up appointments to assess patient needs and monitor progress.
- Prescribes medications and treatments to address diagnosed health conditions.
- Interprets clinical data to determine appropriate management and treatment strategies.
- Orders and evaluates diagnostic/laboratory tests, ensuring appropriate follow-up when abnormal results are identified.
- Facilitates ease of access to prevent unnecessary emergency room visits.
- Facilitates referrals to higher levels of care for complex medical concerns beyond the outpatient scope.
- Provides health promotion and disease prevention.
- Conducts routine check-ups and screenings.
- Provides/recommends/refers for routine immunizations/vaccinations.
- Develops and implements individualized patient management plans, ensuring continuity of care through detailed progress documentation.
- Prescribes and monitors medications for physical conditions, evaluating efficacy and managing potential side effects.
- Provides comprehensive education on diagnosis, treatment options (including lifestyle changes i.e. diet, exercise, smoking cessation), medication risks, benefits, and alternatives to support informed decision-making.
- Collaborates with medical staff and clinical teams to uphold quality standards and ensure effective care coordination.
- Coordinates with behavioral health providers and external healthcare teams to optimize patient care and treatment outcomes.
- Participates in peer review processes to ensure clinical excellence and continuous improvement in patient care.
