Ritemed is seeking a full-time Nurse Practitioner to provide on-site clinical services at Concourse Rehab & Nursing Center, located at 1072 Grand Concourse in the Bronx.

This is a facility-based role within a skilled nursing environment. The Nurse Practitioner will manage post-acute and long-term care residents, working closely with attending physicians and facility leadership to support strong clinical outcomes and quality care.

Key Responsibilities
• Provide comprehensive medical care to residents within the nursing home
• Perform assessments, follow-ups, and manage acute changes in condition
• Collaborate with physicians and nursing leadership
• Participate in care planning and interdisciplinary meetings
• Ensure timely and accurate clinical documentation
• Support initiatives to improve quality measures and reduce rehospitalizations

Qualifications
• Active New York Nurse Practitioner license
• Minimum of 6 months of Nurse Practitioner experience required
• Nursing home experience required
• Spanish-speaking preferred
• Strong clinical assessment and decision-making skills

Schedule
• Full-time position
• Shift options: 9:00 AM – 5:00 PM or 3:00 PM – 11:00 PM
