Ritemed is seeking a full-time Nurse Practitioner to provide on-site clinical services at Galloway Rehabilitation and Nursing, located in Galloway, New Jersey.

This is a facility-based role within a skilled nursing environment. The Nurse Practitioner will manage post-acute and long-term care residents, working closely with attending physicians and facility leadership to support strong clinical outcomes.

Key Responsibilities
• Provide comprehensive medical care to residents within the nursing home
• Perform assessments, follow-ups, and manage acute changes in condition
• Collaborate with physicians and nursing leadership
• Participate in care planning and interdisciplinary meetings
• Ensure timely and accurate documentation
• Support quality initiatives and rehospitalization reduction efforts

Qualifications
• Active New Jersey Nurse Practitioner license
• Minimum of 6 months NP experience required
• Nursing home experience preferred
• Strong clinical assessment and decision-making skills
• Ability to work independently within a structured clinical model
