As a Nurse Practitioner – Wound Care Team Leader, you will play a critical role in advancing our wound care program within the assigned market. This leadership position requires clinical expertise in wound care, strong team collaboration abilities, and a passion for improving patient outcomes. You will oversee clinical outcomes, support wound care nurses, and work closely with interdisciplinary teams to ensure excellence in in-home wound care delivery.

Responsibilities:

  • Provide expert wound care assessments, treatment planning, and management in the home setting following evidence-based guidelines and company protocols.
  • Lead and mentor wound care nurses within the assigned market to promote best practices and enhance clinical outcomes.
  • Collaborate with interdisciplinary teams including physicians, case managers, and other healthcare professionals to coordinate patient-centered care.
  • Monitor clinical quality, compliance, and performance metrics to ensure adherence to regulatory and company standards.
  • Serve as a clinical resource and subject matter expert for wound care within the market
  • Participate in continuous quality improvement activities, product evaluations, and new treatment protocol development.
  • Engage with patients and caregivers to provide education, support, and ensure satisfaction with care delivery.
  • Demonstrate leadership by fostering a culture of teamwork, accountability, and clinical excellence.
