Job Title: Part-Time/PRN Occupational Therapist (Home Health)

Responsibilities
Provide occupational therapy evaluations and treatments based on physician-approved care plans.
Develop and adjust individualized therapy plans to meet patient goals.
Educate patients and caregivers on exercises, safety, and daily living strategies.
Collaborate with nurses, physicians, and the care team to coordinate care.
Complete accurate and timely documentation using electronic charting systems.
Follow all safety, infection control, and agency protocols.

Qualifications
Graduate of an accredited Occupational Therapy program.
Active Indiana OT license required.
At least one year of clinical experience; home health experience preferred.
Strong documentation and communication skills.
Comfortable working independently in a home-based setting.
