Job Description

Provides social work services for patients and their families and collaborates with patients, families, and the healthcare team; assists families by providing resources/referrals for community assistance including financial resources. Provides crisis intervention and follow up services.  Exhibits the F.I.R.S.T. values (Friendliness, Innovation, Respect, Service, and Trust).

WORKING CONDITIONS, HAZARDS AND PHYSICAL EFFORT:

Works in typical office setting. Must be able to work under pressure and meet deadlines. May require working evening and weekend shift with overtime as necessary. Ability to move or position medium weight files under 10 lbs. The tasks of this job do not involve exposure to blood, body fluid or tissue.

 

 

CONTACTS WITH OTHERS:

Works with physicians, Associates, Administrative staff, patients, families, community and referral agencies, and other healthcare providers. Must be able to communicate effectively with the Medical Staff. Should be pleasant, conscientious, objective, tactful, diplomatic and responsible, and possess skills necessary to work effectively with other disciplines.

 

EQUIPMENT USED/SPECIAL SKILLS REQUIRED:

In-house computer, departmental computers, copy machine and fax machine. Electronic medical record keeping knowledge preferred. Meditech experience preferred. Requires independent judgment and initiative. Utilizes communication skills with a high degree of effectiveness in working with patients, families, physicians, hospital personnel and referral agencies. Possesses skills necessary to plan and coordinate a social service program and to work effectively with other disciplines.  Should possess knowledge of community, social, health and welfare programs.
