Position Summary

Blue Ridge Care is seeking a compassionate and experienced Nurse Practitioner to provide primary care services within our PACE program.

Under the supervision of the Medical Director, this role performs comprehensive assessments, manages ongoing care, and partners closely with the Interdisciplinary Team (IDT) to support frail elderly participants.

This position plays a vital role in developing care plans, treating acute and chronic conditions, and providing education to participants and caregivers while ensuring high-quality, coordinated care.

Essential Duties and Responsibilities

  • Perform comprehensive history and physical assessments for new participants

  • Conduct semi-annual and as-needed reassessments and update plan of care

  • Collaborate with the IDT to develop and implement individualized care plans

  • Evaluate and treat participants during acute and chronic illness

  • Coordinate referrals to specialists in collaboration with the Primary Care Physician

  • Maintain accurate and timely participant medical records and documentation

  • Provide education to participants, caregivers, and staff

  • Participate actively in IDT meetings and provide clinical input

  • Coordinate care across multiple care settings and external providers

  • Support quality improvement initiatives and identify care enhancements

  • Serve as a clinical resource to team members

  • Ensure compliance with all regulations, policies, and safety standards

  • Maintain confidentiality and follow infection control procedures

  • Participate in continuing education and required trainings

  • Perform other duties as assigned

Education and Experience

  • Graduate of an accredited school of professional nursing required

  • Licensed as a Registered Nurse (RN) and Nurse Practitioner in the state

  • Certification as a Nurse Practitioner by a nationally recognized organization

  • Geriatric Nurse Practitioner (GNP) certification with prescriptive authority preferred

  • Minimum of 2 years of experience working with frail or elderly populations required

  • Experience in a multidisciplinary setting preferred (hospital, nursing home, or community-based)

  • 2+ years of experience as a Nurse Practitioner in geriatrics preferred

  • Current CPR certification required

  • Valid driver’s license, reliable transportation, and proof of insurance required

  • Must meet immunization and communicable disease requirements

Skills and Knowledge

  • Strong understanding of geriatric and chronic care management

  • Knowledge of home- and community-based care

  • Strong communication and interpersonal skills

  • Ability to work collaboratively in an interdisciplinary team

  • Strong organization and time management skills

  • Ability to manage changing priorities in a dynamic environment

  • Proficient in EMR systems and general computer use

  • Ability to read, write, and speak English fluently

Work Environment & Physical Requirements

  • May be exposed to healthcare-related risks (bodily fluids, communicable diseases, etc.)

  • Ability to stand, walk, bend, and lift up to 50 pounds

  • Ability to push/pull up to 150 pounds with proper equipment

  • Requires manual dexterity and coordination

  • Work environment may be fast-paced, noisy, and emotionally demanding
