Please note that this role is per diem with a rate of $160 - $180 per visit.

Responsibilities

  • Evaluate patients’ functional status, mobility, muscle strength, gait, and ROM to develop individualized treatment plans.
  • Deliver physical therapy services in accordance with the physician’s plan of care and California’s Physical Therapy Practice Act.
  • Educate patients and their families or caregivers on therapeutic exercises, equipment use, and recovery goals.
  • Collaborate with physicians and the care team—reporting changes in patient condition and participating in case conferences.
  • Supervise Physical Therapy Assistants and, when PT is the sole skilled service, oversee Home Health Aide care bi-weekly.
  • Manage documentation including clinical progress summaries and discharge planning.
  • Identify needs for additional home health services and coordinate referrals as appropriate.
  • Perform other duties as required for patient services.
