Department: Population Health

Shift/schedule: Full Time (40 hrs/wk)

GENERAL SUMMARY

Works under the supervision of the Adm. Director of Population Health. The Population Health Assistant's primary function is to perform a variety of clerical activities to meet the day-to-day operations of the Population Health departments, to include the Care Center, Community Health, and Value-Based Care, and assists with coordination of services. Assumes responsibility for various projects, prepares various reports, maintains departmental files, prepares for meetings/trainings. Performs other duties as assigned.

QUALIFICATIONS

Education:

  • High School Diploma or successful completion of an equivalent High School Exam Required
  • Computer courses preferred

Licensure:

  • None

Experience:

  • Working knowledge of Microsoft Word, PowerPoint and Excel required.
  • Previous clerical, computer, and/or similar office experience preferred

    JOB SPECIFIC DUTIES AND PERFORMANCE EXPECTATIONS

    The following is a summary of the major job duties of this job. Other duties may be performed, both major and minor, which are not mentioned below. Specific activities may change from time to time. 

    1. Serves as clerical support for the Director of Population Health and various team meetings, and activities; scheduling and taking accurate minutes, preparing PowerPoints, excel spreadsheets, word documents, and maintaining files.

    2. Assists and supports various projects for the departments, including Population Health, the Care Center, Community Health, and Value-Based Care.

    3. Performs data entry, types, proofs, and edits department correspondence (i.e. memos, letters, minutes, and policies) accurately and in a timely manner.

    4. Utilizes computers for data, information retrieval, and printing reports.

    5. Assists with the coordination and collaboration of community relationships, events, and projects.

    6. Performs general day-to-day office duties, including ordering and organization of departmental supplies, gathering information, and preparing reports as required.

    7. Maintains the schedule for the Administrative Director of Population Health

    8. Makes travel arrangements for the department.

    9. Assists in preparation, organization, and follow-up documentation as needed for Quality & Value-Based Care programs.

    10. Assists with organizing, creating, designing, and producing reports and presentations for Population Health, the Care Center, Community Health, and Value-Based Care.

    11. Assists and provides support for grant reports and regulatory submissions as needed.

    12. Assists and provides support in collecting, organizing, and summarizing data.

    13. Performs other duties as assigned.
