Job Description

Aevum Behavioral Health is seeking a compassionate and skilled Family Nurse Practitioner (FNP) for a flexible part-time position, 1–3 days per week. Join our dedicated team and provide in-person, patient-centered primary care to adult and geriatric patients across assisted living facility (ALF) and outpatient clinic settings. Qualifications:

       Active and unrestricted license as a Family Nurse Practitioner (FNP) in the state of Virginia.

       Board certification as a Family Nurse Practitioner is required.

       Clinical focus and experience with adult and geriatric populations; pediatric care is not within the scope of this role.

       Experience providing primary care in ALF, long-term care, skilled nursing, or outpatient clinic settings preferred.

       Proficiency in chronic disease management, preventive care, and geriatric-specific conditions (e.g., dementia, polypharmacy, fall risk, functional decline).

       Strong understanding of medication management for complex, multi-morbid adult and geriatric patients.

       Experience using electronic health records (EHR) systems.

       Ability to work collaboratively within interdisciplinary care teams.

Responsibilities:

       Conduct comprehensive primary care assessments and develop individualized care plans for adult and geriatric patients.

       Manage acute and chronic medical conditions common in aging populations, including hypertension, diabetes, COPD, heart failure, and cognitive decline.

       Provide medication management, prescription refills, and ongoing monitoring for complex medical regimens.

       Perform routine and preventive care visits, including wellness exams, screenings, and immunizations.

       Conduct rounds at assisted living facilities and provide follow-up care in the outpatient clinic setting.

       Collaborate with behavioral health clinicians, social workers, and facility staff to ensure coordinated, whole-person care.

       Communicate effectively with patients, families, and caregivers regarding diagnosis, treatment options, and care transitions.

       Maintain accurate, timely, and thorough documentation in EHR systems.

       Adhere to all regulatory requirements applicable to ALF and outpatient clinical environments.
