Alternative Family Services is looking for a Program Coordinator to join our San Leandro location supporting ECM. The Program Coordinator is responsible for the coordination of client and family services. The Program Coordinator will support various agency programs and may include social services, mental health, educational and prevention programs. Alternative Family Services is a Foster Family, Adoption and Mental Health Agency that has been serving abused and neglected children in our community since 1973. Our mission is to support vulnerable children and families in need of stability, safety, and well-being in communities. We provide a wide range of foster, adoption, mental health and transition-aged youth services in English and Spanish. We have eight locations throughout the Bay Area and over 200 employees. Driving Requirements
This position requires the employee to work out in the community and within foster homes throughout the Bay Area. Thus, position requires employee to possess a valid CA driver’s license and valid automobile insurance with his/her name on the verification card as an insured driver on the policy at all times. Position requires daily access to a car in safe working order.

Qualifications
- High school graduate or GED required
- Minimum 3 years experience in a social service related field, providing services to children or families with complex needs
- Obtain and maintain fingerprint and government and Agency required background clearances
- CPR/First Aid certification
- Able to drive and transport children into a variety of community settings using a personal vehicle that is maintained in safe working order
- Current and valid California driver’s license with an acceptable driving record, current and valid auto insurance showing as an insured driver on the policy and vehicle registration
- Possess a high level of integrity, honesty and strong work ethic
- Able to read and write fluently in English
- If applying for Spanish-speaking pay differential, must be able to meet linguistic needs of families
- Possess strong verbal and written communication skills
- Able to lead the facilitation of group meetings
- Able to apply common sense understanding to carry out instructions furnished in written, oral or diagram form
- Ability to serve a diverse client and family population with cultural awareness
- Able to maintain a professional demeanor in a stressful environment
- Able to interact calmly and professionally with clients who may act out due to behavioral disabilities
- Able to interact with co-workers, county representatives, and other treatment team members as part of a cooperative team
- Able to work with minimal direct supervision and manage time and prioritize workload

Responsibilities
- Conduct outreach and engagement activities to facilitate linkage to the services for various agency programs. Outreach and Engagement may consist of phone calls, mailed information, and field visits
- Serve as the day-to-day contact for clients and cases assigned to address questions about the status of referrals or cases assigned
- Coordinate with internal and external services for AFS clients
- Supervision and oversight of program (ECM, ISFC, etc.) cases
- Collaboration with AFS QM department
- Offer services where the client lives, seeks care, or finds most easily accessible, including office-based, telehealth, or field-based services
- Liaison with county and community representatives to establish and build open communications and relationships
- Learn and apply various evidence-based practices based on the populations’ needs to support referrals, clients and families
- Contribute to a positive team culture
- Lead and participate in group programming, staff meetings, support groups and training sessions
- Provide services via methods consistent with the agency’s standards and policies
- Attention to detail to monitor client assignment and complete tasks in line with required timelines
- Ability to engage clients in multiple modalities, including text, email, virtual and in-person
- Escalate issues appropriately and provide input into continuous quality improvement efforts
- Review and approve documentation for quality of completion and timeliness ensuring all written material and computer data entry is completed in a timely manner
- Attend and participate in meetings as assigned
- Maintain client charts to ensure compliance with Contractual/Medi-Cal/county/state regulations
- Visit client sites and transport clients using personal vehicle that is maintained in safe working order into a variety of community settings
- Provide emergency and crisis response. Evenings and weekends may be required
- Other duties as assigned
