Program: Union PATH

*This role is $24 an hour*

Location: 615 North Broad Street, Elizabeth NJ

Work hours:  Monday-Thursday 9:00am-5:00pm


Position Overview: 

Provides program support and facilitates communication among the team members and people served. Often serves as the initial point of contact for persons served or other providers calling or visiting the office or HUB. As a member of the team(s), performs general program support functions and housing support duties as assigned with the program.


Essential Job Functions/Responsibilities:

Team Support:

  • Ensures a friendly wellness and recovery-oriented office environment, and rapid response when meeting and greeting persons served, family members and stakeholders who visit the office.
  • Creates a welcoming trauma-informed customer service environment in the office and HUB.
  • Participates in team meetings as assigned, including individual and team supervision. Actively seeks out monthly supervision from supervisor.
  • Provide admission information for program or warm handoffs to other Bridgeway programs for persons seeking Bridgeway services for self or family members.

Shelter Plus Care:

  • Provides recipients of Shelter Plus Care with yearly written, verbal and/or email notifications of recertification requirements to maintain compliance with HUD.
  • Follows up with any recipients not active in services by completing a face to face visit in the field and documents attempts quarterly.
  • Meets with recipients on an annual or as needed basis to collect recertification documents. Problem solves with individuals with housing/landlord related issues.
  • Works collaboratively with Union PATH Team Leader and team, consisting of Elizabeth and Plainfield Housing Authority along with Union County Chart Compliance.
  • Assists team leader with managing service match, submitting reports on a monthly basis. 

Homeless Diversion:

  • Answers phones for Diversion staff and facilitate giving information about services
  • Enters all calls into the Diversion call log and assists with setting up appointment times.

Housing Supports:  

  • Works with the Peer Housing Manager in order to submit paperwork in a timely manner to the NJ MCO’s for authorization 
  • Track authorizations and submit for renewals as needed. 
  • Enter all contacts data in to HMIS

Data Entry and Reporting:

  • Enters all billing information in accordance with agency timeline
  • Enters all statistical data collection and prepares monthly and quarterly reports in accordance with agency timeline
  • Track all required documentation required through spreadsheets, chart auditing to help team maintain compliance. Includes due dates of housing recerts, IRP’s, assessments, Med Ed, and Initial/Annual documents
  • Track and report units of service (weekly) o    Assists in the collection of program data Billing:
  • Monthly verification of insurance eligibility for individuals and reporting changes to billing supervisor/team leader
  • Maintains Program’s supply inventory, prepares vouchers for payment
  • Enters all billing information in accordance with agency timeline to include use of Traverse PO System and Concur System/maintenance of receipts
  • Daily tracking and reporting of units of service, and billing through NJMHAPPS as applicable

Coordination of All Aspects of Health and Safety:

  • Inspection of all Agency vehicles (repairs/routine maintenance/inspections)
  • Conducts and documents all required inspections and drills outlined in Health and Safety Policies (see policy 6004-A)
  • Compliance with internal/external policies, regulations, laws and high ethical standards.

Coordinates Routine Administrative Program Activities:

  • Management of charts in electronic health record
  • Appointments and meetings for staff and maintains calendar as assigned
  • Attends team meetings and in-service trainings as assigned

Maintains:

  • Management of information systems including HMIS, and the and distributes information/reports to the other team members and Director as assigned.
  • Confidentiality of mental health program information, records, etc.

Supply Management:

  • Manages the program’s supply inventory in collaboration with program staff and Director
  • Communicates with finance department to ensure ordering of supplies is within budget and using the most appropriate vendor

Decision Making/Customer Service/Communication:

  • On a regular and continuous basis, exercises sound judgment and assumes responsibility for decisions, consequences and results having an impact on people, costs and/or quality of service
  • Answering, screening and routing telephone calls including routine and crisis calls from people served by the program
  • Provide admission information for program or warm handoffs to other Bridgeway programs for persons seeking Bridgeway services for self or family members
  • Prepares and answers routine team correspondence
  • Exchanges non-routine information using tact and persuasion as appropriate, requiring excellentverbal and written communication skills
  • Willing to learn and competently utilize all current and future computer applications and technology as related to the position to include operation of agency computers, smart phones, phone systems, email platforms, electronic calendars, electronic training modules, electronic health records and electronic data systems used by the agency.

Requirements:

Education/Certifications:

    Associates degree in Psychology, social work, or counseling related field.

Experience

    A minimum of two years administrative work experience

Driver’s License/Vehicle

    Valid driver’s license required

    No more than one moving violation within the past 12 months

    Vehicle required

Subcommittee and Mandated Trainings Statements:

All employees are encouraged to participate on an agency committee, on a rotating basis.

Mandatory trainings are to be completed during regularly scheduled working hours. It is the employee’s responsibility to attend mandatory trainings when they are offered by the agency and to complete mandatory web- based training in a timely manner. Employees failing to attend mandatory trainings are required to make up the training on their own time and at their own expense, i.e., CPR training.
