Job Description

Psychiatric Nurse Practitioner  (Regular Full-Time, Hourly)
Region: South San Diego
Career Type: Nurses

Hours: This is an exempt, salaried position. 

Pay Rate: $227,136 per year

Education: Master's of Science degree in Nursing (MSN) or Doctor of Nursing Practice (DNP)

Licensure: Current California Psychiatric Nurse Practitioner license required

As a Psychiatric Nurse Practitioner, you will:

  • Provide medication management and support services to our ICM (Intensive Case Management) clients
  • Assist multidisciplinary team with crisis intervention and psychiatric assessments
  • Ensure that clients and family members are educated and informed about their medications and illness management
  • Monitor health care needs of the clients and obtains assistance from other health care providers as necessary
  • Ensure all medications are managed and stored appropriately as defined by local and state regulations
  • Provide services in the community or client’s home, including administration of medication

Additional Qualifications and Skills

  • One year of PHNPM experience preferred
  • Minimum of one year of experience working with a mental health population, especially with individuals with severe and persistent mental illness and co-occurring disorders is strongly preferred
  • Ability to physically and mentally able to assist in emergency situations is required
  • Bilingual Spanish preferred but not required
  • Demonstrated knowledge of County Health and Human Services requirements including documentation is preferred
  • A valid California driver’s license in good standing, personal vehicle, and proof of insurance is required to fulfill the essential functions of this position.
  • Ability to pass criminal/background clearance

Intensive Case Management (ICM) is a community-based program serving adults living with serious mental illness who require a higher level of support to achieve stability and independence. The program focuses on individuals with complex needs who may be at risk of hospitalization, homelessness, or disengagement from care.

ICM utilizes a multidisciplinary, team-based approach to provide comprehensive, individualized services in the community. Staff work closely with clients to coordinate care, support access to housing and benefits, assist with medication and treatment engagement, and build skills to promote recovery and independence.

With smaller caseloads and a strong emphasis on outreach, ICM allows staff to develop meaningful relationships with clients while delivering high-quality, person-centered care. This model is designed to improve overall outcomes, reduce crisis utilization, and support long-term community integration.
