Full-Time Psychiatric Nurse Practitioner (PMHNP) – In-Office Position

T.O.N.E. Home Health Services is seeking a full-time Psychiatric Mental Health Nurse Practitioner (PMHNP) to join our team in an in-office role supporting behavioral health and psychiatric oversight within the home health setting.

This position will work closely with our clinical and administrative teams to assist with psychiatric evaluations, medication management support, care coordination, and behavioral health guidance for home health patients.

We are looking for someone who is organized, technology-comfortable, responsive, and able to collaborate effectively within a fast-paced healthcare environment.

Farmington Hills, Michigan

Schedule:

Responsibilities:

• Conduct psychiatric and behavioral health evaluations


• Provide medication management and treatment recommendations


• Collaborate with nurses, therapists, social workers, and physicians


• Support patients experiencing anxiety, depression, dementia-related behaviors, mood disorders, and other psychiatric conditions


• Participate in care coordination and interdisciplinary discussions


• Complete timely documentation within Kinnser/WellSky EMR


• Assist with behavioral health treatment planning and oversight


• Communicate effectively with staff, patients, and families
