Job Description

Description

Provides professional services for the community in the delivery of health promotion/disease prevention activities and case management primarily focused in Family Health.

 

Primary Duties & Responsibilities:

·         Provides Professional Public Health services to individuals and families in the community.

·         Participates in health promotion clinics and related activities as assigned.

·         May conduct disease outbreak follow-up and control as assigned.

·         Participates in public health emergency preparedness planning.

·         Completes assessments for eligibility programs offered through state waivers

·         Provides care coordination for various health plans as contracted for senior clients

·         Ability to provide immunizations to all ages when necessary.

Requirements

·         Ability to establish and maintain cooperative working relationships with other staff members, representatives of other agencies, clients, and the general public. 

·         Ability to utilize effective teaching methods in relating to clients, families, groups and staff. 

·         Computer experience required.

 

Minimum Qualifications:   

·         Current licensure as a Registered Nurse in the State of Minnesota. 

·         Subject to criminal history background check.
