Subject: Registered Nurse
Position summary:
The Registered Nurse is responsible and accountable for providing optimal care and services to patients with end-stage renal disease, who are dependent upon chronic dialysis therapy for life. As a primary care nurse, the RN is accountable for providing total, comprehensive, continuous patient-centered care for his or her own caseload of patients. To provide opportunities for the patient to participate in his or her own care whenever possible. To formulate a comprehensive care plan for the patient based on nursing assessment. Must be able to give direction, observe the nursing process of other team members, and assume a resource and clinical role. Must be able to problem solve with little assistance and handle stressful situations calmly.
Qualifications:
- Current California license to practice as a registered professional nurse
- Current CPR
- Supervisory experience preferred
- Dialysis experience desirable
- Knowledge and understanding of Nurse Practice Act
- Ability to write and direct staff in the use of standards of nursing care.
Duties and Responsibilities:
- Nursing Care Planning
- Inform and establish therapeutic relationship with patients.
- Inform and establish liaison with all other health care team members.
- Obtain a nursing history and complete nursing assessment form within the first 30 days of admission to unit.
- Participate in completion of the Comprehensive Patient Assessment with other members of the multi-disciplinary team and the patient.
- Continually assess patient care needs and modify nursing care plan accordingly and communicate plan to health care team members who are involved with the patients care.
- Assume the duties and responsibilities of Charge Nurse as required in his or her absence.
- Include patients and their families in all aspects of planning and teaching.
- Patient Care Management and Responsibilities:
- Assume daily care on patients
- Supervise and delegate care of patients
- Frequent rounds with primary Physician and or Nurse Practitioner
- Initiate and terminate dialysis treatments
- Monitor all daily log sheets
- Check on all medical orders
- Monthly update of patient home medication lists
- Keep nursing care plan up to date
- Establish short and long term goals for patients
- Medical Record
a. Be familiar with all aspects of patient record
b. Be familiar with all laboratory data
c. Obtain a nursing history for all primary patients
d. Complete monthly nursing notes/summary on assigned patients
- Assist patient in making clinic appointments as needed.
- Participate in assigned CQI projects
- Perform Acute Hemodialysis treatments at assigned hospitals as needed.
