POSITION SUMMARY



The HEDIS Nurse at MiSide Health supports quality improvement efforts within the Certified Community Behavioral Health Clinic (CCBHC) model. This role focuses on identifying and closing gaps in care for youth and adults with behavioral health and co-occurring medical conditions, ensuring compliance with DWIHN, MDHHS, and NCQA standards. The HEDIS Nurse collaborates across clinical teams to support integrated, whole-person care.





KEY RESPONSIBILITIES




  • Review and abstract clinical data in MiEMR to support HEDIS and CCBHC measures

  • Identify and track gaps in care (labs, screenings, follow-ups)

  • Coordinate with psychiatry, nursing, access, and care teams to ensure completion of services

  • Support outreach efforts to improve client engagement and compliance

  • Monitor quality dashboards and performance metrics

  • Ensure accurate documentation for compliance and audits

  • Provide staff education on HEDIS measures and workflows

  • Participate in quality improvement initiatives and internal audits
