We are seeking an experienced and dedicated Hospice Registered Nurse (RN) to provide exceptional end-of-life care to patients and their families. The Registered Nurse plans, organizes, and directs hospice care, utilizing community resources and working collaboratively with the interdisciplinary team to meet the physical, emotional, and spiritual needs of hospice patients and families in their home, hospice house, or community setting.
Schedule & Compensation
- Part-Time (.5 FTE)
- Two (2) scheduled day shifts per week
- Two (2) night on-call/triage shifts per week
- $200 per on-call shift, plus hourly pay for patient visits completed while on call
- $40.93 - $56.66 per hour DOE
Responsibilities
- Complete comprehensive patient and family assessments and develop individualized plans of care.
- Provide skilled nursing care, symptom management, medication administration, and medication reconciliation.
- Manage and coordinate care for an assigned hospice caseload.
- Educate and support patients, families, and caregivers regarding disease progression, medications, and comfort-focused care.
- Collaborate with physicians, social workers, chaplains, hospice aides, and other interdisciplinary team members.
- Maintain accurate and timely clinical documentation.
- Coordinate services, equipment, supplies, and community resources to support patient needs.
- Participate in interdisciplinary team meetings and care planning.
- Supervise and support Hospice Aides and Licensed Practical Nurses.
- Participate in after-hours triage, on-call coverage, and weekend responsibilities as assigned.
- Participate in Quality Assessment and Performance Improvement (QAPI) activities and maintain compliance with hospice regulations.
Qualifications
- Current Minnesota RN license in good standing.
- Graduate of an accredited nursing program.
- Minimum three (3) years of acute care nursing experience required.
- Hospice or home care experience preferred.
- Strong assessment, communication, documentation, and critical thinking skills.
- Ability to work independently while collaborating with an interdisciplinary team.
- Valid driver's license, reliable transportation, and current automobile insurance.
