Job Description

PM & AM Shift Open

Rivers Edge Nursing & Rehab are seeking a compassionate and dedicated Registered Nurse (RN) to join our care team. The RN will provide direct resident care, supervise nursing assistants, coordinate with the interdisciplinary team, and ensure compliance with state, federal, and facility standards. This role requires strong clinical judgment, leadership, and a commitment to providing high-quality care to residents.

Responsibilities

  • Deliver and oversee safe, resident-centered nursing care in accordance with care plans and physician orders.

  • Perform comprehensive assessments, monitor changes in residents’ conditions, and report to physicians or nurse management as necessary.

  • Administer medications and treatments accurately while following regulatory guidelines and facility policies.

  • Supervise, train, and support LPNs and CNAs to ensure quality care delivery.

  • Collaborate with the interdisciplinary team to develop, implement, and evaluate individualized care plans.

  • Document resident care, progress notes, and interventions accurately in medical records.

  • Participate in admission, transfer, and discharge processes.

  • Maintain infection control and safety standards, including adherence to universal precautions.

  • Respond promptly and appropriately to emergencies, including providing CPR/first aid when necessary.

  • Participate in care conferences, staff meetings, and ongoing training/education.

  • Uphold resident rights, privacy, and dignity at all times.

Qualifications

  • Current and active Registered Nurse (RN) license in WI

  • Graduate of an accredited nursing program (ADN required, BSN preferred).

  • CPR/BLS certification required.

  • Previous experience in long-term care or skilled nursing preferred but not required.

  • Strong leadership, communication, and critical thinking skills.

  • Ability to work collaboratively in a fast-paced healthcare environment.
