MENTOR Adult Day Health, a member of the Sevita family, provides day programs for elders with complex medical or rehabilitative needs. This allows seniors to obtain quality healthcare while remaining active and involved in the community and continuing to live in their own homes. Our programs are staffed with qualified, experienced licensed nurses and caregivers who are trained to provide individual assistance to each person we serve.

What You'll Do

As a Registered Nurse, you will play a key role in supporting the health, wellness, and quality of life of program participants by:

  • Conducting health assessments and nursing evaluations
  • Assisting in the development and implementation of individualized care plans
  • Reviewing and maintaining nursing documentation, medication administration records, and physician orders
  • Monitoring participant health and proactively addressing medical concerns
  • Administering medications and treatments in accordance with established protocols
  • Promoting health education, wellness initiatives, and preventative care practices
  • Providing training, guidance, and support to program staff regarding medical needs and best practices
  • Collaborating with physicians, specialists, families, and interdisciplinary team members to ensure coordinated care
  • Participating in care planning meetings and providing nursing expertise to support participant outcomes
  • Helping maintain a safe, healthy, and person-centered environment for all participants

 

Qualifications

  • Graduate of an accredited Registered Nurse (RN) program
  • Current Massachusetts RN license required
  • Current CPR and First Aid certification
  • One year of nursing experience preferred, particularly with medically complex adults, seniors, or individuals with disabilities
  • Strong communication, organizational, and critical-thinking skills
  • Bilingual candidates, especially those fluent in Spanish, are encouraged to apply
