Job Description

The Senior Living Advisor is responsible for guiding older adults and their families through the process of evaluating care needs and identifying appropriate senior living and support options, including assisted living, memory care, and in‑home care solutions. This role functions as an independent, objective advisor, helping families make informed decisions that prioritize clinical appropriateness, long‑term stability, and quality of life.

Key responsibilities

  • Conduct comprehensive assessments of clients’ medical, functional, cognitive, psychosocial, and environmental needs, including safety and support systems.
  • Develop individualized care and senior living plans that may include in‑home services, assisted living, memory care, skilled nursing, or hybrid arrangements, with clear rationales and priorities.
  • Educate clients and families on the continuum of senior care options, levels of care, eligibility criteria, and typical costs, helping them understand trade‑offs and timelines.
  • Coordinate and facilitate senior living placement, including identifying appropriate communities, arranging tours, supporting applications, and guiding move‑in and transition planning.
  • Collaborate closely with Unicity Care Management nurses, social workers, and external providers to align recommendations with clinical assessments and ongoing care management plans.
  • Serve as the primary point of contact and advocate for assigned clients and families, maintaining regular communication, providing updates, and helping resolve issues or crises as they arise.
  • Attend care conferences, provider meetings, and family meetings as needed to represent the client’s best interests and ensure clarity around care plans and senior living strategies.
  • Monitor client status and placement outcomes post‑move and, in collaboration with care managers, recommend adjustments to services or setting when risks, needs, or goals change.
  • Maintain accurate, timely documentation of assessments, recommendations, communications, and outcomes in Unicity’s care management systems in accordance with internal policies and regulatory standards.
  • Build and maintain a vetted network of senior living communities and related resources (home care agencies, elder law, benefits advisors) while upholding Unicity’s non‑commissioned, objective advisor standard

Requirements

Qualifications:

  • Minimum 3–5 years of experience working with older adults in senior living, care management, hospital discharge planning, home care, or related geriatric services.
  • Strong knowledge of local senior living options (independent living, assisted living, memory care, CCRC, skilled nursing) and in‑home support resources in northern and central New Jersey.
  • Demonstrated ability to perform comprehensive assessments, formulate care recommendations, and communicate complex information clearly to families under stress.
  • Excellent interpersonal skills, with the ability to build trust quickly, manage sensitive family dynamics, and maintain professional boundaries.
  • High level of organization and follow‑through, with comfort managing multiple cases, timelines, and stakeholders simultaneously.
  • Proficiency with electronic documentation and Microsoft 365; experience with care management or CRM platforms is a plus
  • Valid driver’s license, reliable personal vehicle, and ability to travel to client homes, facilities, and provider offices within the service area.
