Job Description

Description

This position is part of a multi-faceted system of support for the CTUIR elder and caregiver population. The primary role of this position is to provide transportation to and from congregate meals at Nicht-Yow-Way Senior Center, deliver meals to homebound Elders and Caregivers, complete janitorial duties at Senior Center, and support the current workforce in maintaining access to elders and caregivers. The Title VI grant work at the Nicht-Yow-Way Senior Center includes driving individual elders and caregivers to obtain and/or access resources. All duties and responsibilities will be performed in accordance with the Yellowhawk Mission and Vision statements.

 

Essential Job Functions:
1.  Deliver meals to homebound elders and caregivers per Title VI regulations with special attention paid to protecting this vulnerable population from influenza, colds and other communicable infections.  
2.  Provide daily transportation to and from the Nicht-Yow-Way Senior Center for congregate meals to elders and caregivers ensuring social interaction and independence for mobile elders.
3.  Janitorial duties along with knowledge of OSHA/safety policies in regards to the workplace/Nicht-Yow-Way Senior Center.
4.  Outreach to elders as assigned by Senior Center Program Specialist I.
5.  Keep confidential, accurate files, data and resources on Tribal Elders and their needs.
6.  Answers the telephone, greets individuals in person while using active listening skills to assess client needs. 
7.  Maintain a positive working relationship with Yellowhawk, CTUIR, and Community Members.
8.  Maintain confidentiality in accordance with Yellowhawk Personnel Manual and the Health Insurance Portability and Accountability Act of 1996 (HIPAA), and Administration for Community Living Title VI guidelines. 
9.  Be sensitive to patients but maintain professional boundaries.
10. Comply with local traffic regulations to ensure the safety of the Elders being transported.
11. Must inspect Nicht-Yow-Way Senior Center vehicles weekly for preventative maintenance needs.
12. Maintain vehicle log of elders and caregivers being transported, record mileage and vehicle maintenance.
13. Other duties as assigned.

Requirements

Minimum Qualifications:

1.   Must have a High School Diploma or equivalent.
2.  Must have First Aid/CPR certification and Food Handlers Card within 30 days of employment.
3.  Must be able to recognize and understand vital signs (body temperature, blood pressure, pulse rate and respiration rate).
4.  Must be up to date and maintain immunizations required for position and safety.
5.  Represents Yellowhawk with a respected and viable presence in the community.  
6.  Must possess a valid driver’s license and maintain insurance requirements to operate General Services Administration vehicles.
7.  Must submit to and pass a reference and criminal background check.  
8.  Must submit to and pass a pre-employment drug and alcohol screening.


Preferred Qualifications:
  1.  Experience working with and caring for Elders.
  2. Knowledge and experience working in Tribal communities.
