Performs a comprehensive psychosocial assessment that reflects an advanced understanding of physical and mental illnesses and psychosocial issues. Demonstrates ability to perform a comprehensive psychosocial assessment which examines presenting problems as well as relevant bio-psychosocial and environmental factors. Completes and records social work assessments within departmental time and format standards. Formulates a diagnosis using the current DSM criteria and develops an initial treatment plan, addressing identified needs, problems, and areas requiring further evaluation. Coordinates referrals when indicated to the appropriate internal or community resources. Functions independently in providing psychotherapy to treat mental disorders, alleviate emotional distress, and facilitate adaptive behavior, personal growth and development. Demonstrates clinical expertise to achieve therapeutic outcomes with all patients through the use of age-appropriate interventions that may include individual, group and family treatment modalities. Evaluates patient's progress toward treatment goals and revises the plan of care as needed. Documents in a clear, consistent and timely manner effects of interventions, and patient progress on established forms using accepted format. Demonstrates capacity to form a therapeutic alliance with patients and families. Exercises clinical judgment and obtains consultation/supervision on a scheduled/needed basis. Collaborates with the multidisciplinary team in the development and ongoing evaluation of the patient's treatment plan. Provides input regarding the treatment plan which reflects an advanced knowledge of psycho therapeutic interventions and an understanding of age-appropriate developmental needs. Demonstrates in social work plan of care adequate assessment of the needs and perceptions of the patients and their family members. Participates in regular multidisciplinary team rounds and reviews discussing progress towards goals and appropriate revisions. Evidences knowledge of the principles of leadership and effective team building in interactions with other team members. Coordinates patient discharge planning process. Complies with required standards for medical/legal responsibilities. Accountable to continuously improve and develop in the profession of social work and to support the continuous improvement of the department and institution. Maintains professional standards and conduct. Serves as a resource to nursing, medical, allied health, and administrative staff in the areas of consultation and education. Promotes the development of research-based practice.
Qualifications
Master’s degree in Social Work or related, along with licensure to practice independently at the clinical level as required by applicable state requirements, and 1 year of experience required.
Skills: Previous Social Work experience in healthcare;
Experience in rural healthcare;
Advanced knowledge of mental illness, personality development, and types of treatment;
Skills in conducting individual, group, and family therapy;
Clinical/Diagnostic assessment skills;
4,000 hours of supervised clinical practice, including 1,800 hours of direct client contact.
