Provide Compassionate Support When Patients Need It Most
At Valley Presbyterian Hospital, our Social Services team plays a vital role in helping patients and families navigate complex medical, emotional, and social challenges. We are seeking a compassionate and resourceful Medical Social Worker to provide psychosocial assessments, counseling, discharge planning support, and community resource coordination for patients across the continuum of care.
What You'll Do
- Conduct psychosocial assessments to evaluate patients’ social, emotional, cultural, financial, and family needs
- Provide counseling and emotional support to patients and families coping with acute, chronic, or terminal illnesses
- Assist patients and families in understanding and following medical recommendations
- Coordinate referrals to community agencies and support services
- Collaborate with physicians, nurses, case managers, and interdisciplinary teams to support optimal patient outcomes
- Support discharge planning for patients with complex psychosocial needs
- Respond to referrals, emergency requests, and patient care needs in a timely manner
- Maintain accurate, thorough, and compliant documentation
- Participate in quality improvement initiatives, staff meetings, committee work, and departmental projects
- Assist with orientation and training of new staff as needed
- Provide on-call coverage and weekend/holiday support based on departmental needs

Experience
- Minimum two (2) years of hospital social work experience preferred
Education
- Master's Degree in Social Work (MSW) required
Requirements
- Management of Assaultive Behavior (CPI or equivalent) certification within 30 days of hire
- LA City Fire Card certification within 30 days of hire
Schedule Expectations
- Available for a minimum of two shifts per month
- Availability for weekends, holidays, and on-call assignments as scheduled
- Holiday coverage to include one major and two minor holidays annually
