JOB SUMMARY:
RESPONSIBILITIES:
- Evaluate, diagnose, and develop individualized treatment plans for residents with speech, language, cognitive, voice, and swallowing impairments.
- Conduct bedside swallowing evaluations and make dietary texture recommendations as appropriate.
- Provide direct therapy services and modify treatment based on resident progress.
- Document evaluations, daily progress notes, and discharge summaries in accordance with facility policy and payer requirements.
- Collaborate closely with nursing, dietary, and therapy staff to ensure coordinated care and optimal resident outcomes.
- Educate residents, families, and staff regarding communication techniques, compensatory strategies, and swallowing precautions.
- Participate in care plan meetings and contribute to interdisciplinary team discussions.
- Maintain compliance with all state, federal, and facility regulations regarding patient care and documentation.
- Supervise speech therapy assistants and students as assigned.
- Promote a positive, supportive, and therapeutic environment in alignment with the facility’s mission and values.
- Other duties as assigned.
REQUIREMENTS:
- Education: Master’s degree in Speech-Language Pathology from an accredited program.
- Licensure: Current and valid Speech-Language Pathologist license in the state of Oregon (or applicable state).
- Certification: Certificate of Clinical Competence (CCC-SLP) from ASHA preferred; CFY candidates may be considered.
- Experience: Previous experience in skilled nursing, subacute rehab, or geriatrics preferred.
- Skills: Strong clinical reasoning, excellent communication skills, team collaboration, and thorough documentation ability.
