Work Ready Employment Case Manager
Job Summary:
This is Signal Centers' innovative workforce development program that provides a unique combination of job training, paid internships and intensive case management. This Case manager will provide intensive case management support to TANF-eligible individuals to prepare them to become highly trained professionals and obtain long-term, stable employment with benefits.
Qualifications:
- Must have a Bachelor's degree in Social Work, Psychology, Human Services, or a related field.
- Experience working with inner-city families in a case management model or experience in workforce development is required.
- Must have a valid Driver's License and reliable transportation with valid insurance per TN state requirements.
- Must be organized and proficient with keeping case management notes and data entry requirements.
- Must be professional in demeanor and able to represent the agency well in the community.
- Must be a team player and able to work cohesively in a team environment.

Job Duties Include:
1. Job Coaching: Once the participant is ready to seek permanent employment, the Employment Case Manager will work to help secure part-time employment while continuing education and training or full-time employment if/when leaving the program.
2. Intensive Model: The Employment Case Manager follows an intensive case management model, working closely with participants with weekly contact with the participant, the employer, and the education and training entity.
3. Barrier Removal: Provides intensive support to eliminate or overcome barriers to success and collaborates with educators, trainers, and employers to troubleshoot and ensure successful employment.
4. Case Management: Carry and maintain a caseload of 20-25 participants, providing intensive case management and resource linkage while maintaining appropriate case notes, conducting necessary assessments, and entering data into the system to track program progress & outcomes.
5. Training: Participates in training, including DHS-provided training on program support, TANF support, and community resources.
6. Community Outreach: Participates in community, partner, and outreach events as assigned.
7. Assessment & Planning: Conduct initial and ongoing assessments of clients’ skills, experience, and employment barriers to develop personalized career plans.
8. Job Readiness Training: Provide hands-on assistance with resume building, interview preparation, and job search strategies.
9. Resource Coordination: Connect clients with essential community support, including housing assistance, transportation, and childcare, to eliminate barriers to work.
10. Monitoring & Support: Conduct regular check-ins to track client progress, provide ongoing mentoring, and ensure successful long-term job retention.
11. Documentation: Maintain accurate, up-to-date client files, service logs, and outcome reports to ensure compliance with funding and agency requirements.
12. Other duties as assigned by the Director.
