JOB SUMMARY The Coder I position is responsible for accurate, coding, abstracting, claims filing, documentation review and claims denial processing working from the appropriate documentation in the medical record.  The Coder must stay up to date on code changes and coding guidelines to assure quality and code compliance is met at all times. The Coder has additional combined responsibilities of data quality and insurance representative functions working closely with other members of the HIMS department. II. ESSENTIAL JOB RESPONSIBILITIES<ul><li>Reviews inpatient and outpatient medical records to identify the principal diagnosis and all applicable secondary diagnosis and procedures.</li><li>Use computerized encoding system to facilitate accurate coding according to the appropriate classification system.</li><li>Sequence diagnosis and procedures by following ICD-10-CM &amp; ICD-10-PCS, CPT/HCPCS, UHDDS, Medicare, Medicaid, and other fiscal intermediary guidelines.</li><li>Will be cross-trained to assist with backlog in any needed focus-coding group.</li><li>Work cooperatively with medical staff and other healthcare professionals in obtaining documentation to ensure optimal hospital payment and accurate data input.</li><li>Prepare workload reports and participates in department continuous quality improvement studies.</li><li>Abstract medical data from the record to complete discharge data abstract on each outpatient.</li><li>Complete and verify diagnostic, demographic and other information for submission to KHDS.</li><li>Review, verify, and initiate necessary correction processes for data quality review.</li><li>Participate in medical record documentation auditing to monitor physician compliance with regulatory requirements.</li><li>Communicate and advise other hospital personnel on coding and DRG assignment.</li><li>Meet established quality and productivity standards.</li><li>Adhere to all hospital and departmental policies, procedures and regulations, including attendance.</li><li>Perform other related duties as assigned or requested.</li><li>Requires ability to concentrate and maintain accuracy in spite of frequent interruptions and/or distractions, sit for long periods.</li><li>Must be able to follow instructions and use sound judgment.</li><li>Requires close mental and visual attention to details, as well as excellent verbal and written communication skills.</li><li>Able to handle frustration and interactions with others in a professional manner.</li><li>Requires self-motivation to complete work assignments in a timely, accurate manner.</li><li>Maintain ongoing registration and continuing education for applicable credentials</li><li>Performs other duties as needed or assigned.</li><li>Regular and reliable attendance is an essential function of this position</li></ul>III. Develops and conducts education and training to relevant stakeholders as a result of audits.</li><li>Assists with the risk assessment deployment, performance and analysis. Assists in the development, implementation, and maintenance of the annual audit plan performed by third-party vendors. Reviews and appraises the soundness, adequacy, and application of financial and operating controls.</li><li>Assesses the efficiency of resource use. Identifies wasteful expenditures, over payments, revenue enhancements and initiates recovery. Coordinate and oversee efforts to recover erroneous payments made because of misrepresentative billing, fraud, or abuse. Makes recommendations for corrective actions and implements corrective actions when requested.</li><li>Assists with interviews and obtains written statements. Calculates the dollar value of losses. Documents the conclusions reached. Recommends specific steps to prevent recurrence of similar events.</li><li>Prepares statements and either verbal or written reports/updates. Produces documentable audits and investigation findings. Conducts audits to identify patient care and satisfaction issues and makes recommendations for improvement.</li><li>Prepares audit reports; presents findings in exit conferences as appropriate. Communicates to relevant parties the results of audit activity. As appropriate, facilitates development of an action plans, and monitors progress, via continuing audits, and coordinating, education, until satisfactory improvement is achieved.</li><li>Participates in performance/quality improvement activities and patient safety activities. Assists in maintaining compliance with all regulatory agencies. Trends Integrity Helpline calls, Compliance Communication Log, revenue and coding compliance issues, Education Log and other sources to identify education opportunities.</li><li>Serves as a liaison between the Charge and Payment Compliance Coordinators, clinical departments, and physicians, Clinic Coding Services, and the Revenue Cycle team.
