The option of a flexible 40-hour workweek is possible within certain teams.&nbsp;Responsibilities:&nbsp;<ul><li>Evaluate, diagnose and treat patients of all ages and backgrounds to help them optimize function, alleviate pain, and enhance quality of life.</li><li>Provide culturally competent care to a diverse and under served patient population, demonstrating sensitivity to language, cultural practices, and other socioeconomic factors that may impact health and outcomes.</li><li>Develop treatment plans and continuously update individualized treatment and be able to modify treatments when appropriate to achieve these goals. &nbsp;</li><li>Evaluate and record patient progress, modify treatment plans, and try new treatments if required.&nbsp;</li><li>Instruct patients and teach them therapeutic exercises, stretches and a range of motions, flexibility, coordination, and balance to improve their condition.&nbsp;</li><li>Conduct thorough initial evaluations to determine appropriateness for physical therapy, identify impairments, and recommend appropriate plan of care or referral if outside scope.</li><li>Participate in preparing physical therapy patients to transition to the gym once their physical therapy treatment is complete.&nbsp;</li><li>The therapist will only perform interventions within their scope of practice in which they are competent and/or receive advanced continuing education.&nbsp;</li><li>Provide ongoing education and instruction through a written home program to the patient and caregiver.</li><li>Support clinic growth by participating in outreach efforts including patient follow-up calls, scheduling coordination, community marketing initiatives, and collaboration with referring providers.</li><li>Maintain an active caseload of approximately 60+ patient visits per week while ensuring high standards of care and patient satisfaction.</li><li>Serve as a clinical instructor and mentor to physical therapy students, supporting their development in evidence-based practice and professional conduct.</li><li>Perform other duties outside of job description as assigned</li><li>Maintain patient confidentiality&nbsp;</li><li>Communicate with an interdisciplinary team (including primary care providers, community health workers, care managers, administrative staff, etc.) to ensure consistent care to address all aspects of health.&nbsp;</li><li>Participate in regular metric reporting as a team and comply with federal and state physical therapy regulations.&nbsp;</li><li>Collaborate with HR and follow policy and procedures in on boarding physical therapy students and/or interns ensure compliance of students/interns in all required training&nbsp;</li><li>Provide and supervise growth and learning opportunities for physical therapy students and/or interns&nbsp;</li><li>Work with credentialing team on all aspects of information as needed&nbsp;</li><li>Aid in the opening and closing of the gym facility &nbsp;</li><li>Assist in supporting another therapist&rsquo;s clinical caseload as needed.</li><li>Must remain current in best practices in industry and licensure&nbsp;</li><li>Completes required continuing education requirements as mandated by state licensing. &nbsp; &nbsp;</li><li>Chart and track client progress in the EHR (Medent) system</li></ul>&nbsp;Qualifications:                  &nbsp;<ul><li>Doctor of Physical Therapy degree and be licensed to practice in NYS</li><li>Current CPR/AED and First Aid certification or will obtain through Jericho Road Community Health Center</li></ul>&nbsp;Rate:&nbsp;$68,000-$78,000 salary. Individual compensation is based on various factors unique to each candidate, including skill set, experience, qualifications, and other position related components.&nbsp;Job postings are not intended to be an exhaustive list of duties.
