Your Role:

The Physical Therapist is responsible for evaluating, planning, and carrying out individualized treatment programs for home health patients while coordinating care with the broader rehabilitation and clinical team.



Key Responsibilities:

• Provide effective patient care using approved therapy techniques and procedures

• Evaluate patient condition and establish measurable treatment goals

• Develop and implement individualized treatment plans

• Supervise and coordinate care with PTAs and HHAs

• Document patient progress, updates, and treatment notes accurately and on time

• Educate patients and families on exercises, mobility, and use of adaptive equipment

• Evaluate home environments and recommend modifications to improve safety and independence

• Ensure all services meet HIPAA standards for confidentiality

• Coordinate with nursing, social work, and other clinical staff to ensure comprehensive care
