Job Summary

The Palliative Care Nurse Practitioner (NP), practicing under the clinical supervision of a licensed physician, provides comprehensive diagnosis, treatment, and management of patients with serious or progressive illness. Working within established nurse practitioner parameters, the NP delivers hospice and palliative care services focused on symptom management and goals-of-care alignment.

As a primary care provider and hospice/palliative care specialist, the NP conducts patient histories, physical examinations, and assessments, and develops individualized plans of care addressing physical, psychological, emotional, social, and spiritual needs. Care is delivered collaboratively with the interdisciplinary team and in coordination with covering physicians as appropriate.

The NP may provide services across multiple care settings, including hospitals, clinics, private homes, nursing facilities, and assisted living communities, while consistently demonstrating Hospice Savannah’s competencies, values, and service excellence standards. The position is accountable to and evaluated by the Steward Center Medical Director.
