What You’ll Do
• Travel to patient homes and care settings throughout your assigned territory.
• Provide skilled nursing care, including wound care, assessments, and medication management.
• Build relationships with patients and families over time.
• Coordinate with physicians, therapists, and interdisciplinary teams.
• Educate patients and caregivers to support recovery and independence.
• Complete real-time documentation to stay ahead of your day.
What We’re Looking For
• Active Minnesota Registered Nurse (RN) license.
• Experience in home health, med-surg, rehab, or clinical nursing.
• Strong clinical judgment and ability to work independently.
• Organized, self-directed, and comfortable in a field role.
• Reliable transportation and willingness to travel locally.
