Job Description

Part-Time Weekend Hospital Liaison (RN or LPN) | Remote Assessments/Transfer Only | No Hospital Visits

Bridges Health is seeking a Part-Time Weekend Post-Acute Liaison to join our team and support post-acute communities and healthcare providers across Oklahoma.

Note: While this is a remote position, being located in Oklahoma City or the Tulsa metro area is highly preferred.

Available Shift: Friday 4-8PM, Saturday 8:30AM-5PM, & Sunday 8:30AM -5PM

Key Responsibilities

  • Conduct pre-admission screenings and assessments to determine clinical, equipment, and transportation needs.
  • Develop and maintain strong referral development relationship with referral sources.
  • Collaborate with facility Admissions Coordinators to ensure a smooth, patient-centered admission experience.
  • Maintain accurate records of daily activity and referral source interactions within the referral tracking system.

Experience & Qualifications

  • Current and unencumbered Oklahoma LPN or RN license
  • Experience as a hospital liaison
  • Experience in a skilled nursing or skilled rehabilitation setting
