Job Description

Salary Range: $32.25 - $40.31/hour

ESSENTIAL DUTIES, FUNCTIONS AND RESPONSIBILITIES

  • Evaluates, identifies, plans, coordinates, and leads system-wide patient safety activities to prevent patient harm during the process of health care.
  • Reports directly to the Director of Quality Assessment Performance Improvement.
  • Serves as a subject matter expert on healthcare patient safety.
  • The Patient Safety Specialist will collaborate with identified internal and external customers to facilitate patient safety initiatives to reduce unnecessary harm associated with health care. Ensures patients receive safe and quality care while in the hospital.
  • Develops and implements a Patient Safety program to ensure compliance with regulatory healthcare requirements.
  • Conducts surveillance and identifies patient safety opportunities for improvement to minimize preventable harm or injuries.
  • Coordinates, investigates, analyzes, and responds to quality of care and patient safety issues, and events facility-wide.
  • Measures patient safety data for analysis, improvement and reporting.
  • Investigates in detail all patient-related errors and conducts analysis for managing cause of the error.
  • Manages the clinical events (provision of care and patient injuries/falls) and the Root Cause Analysis (RCA) module in the event reporting system. Analyzes entries to address patient safety concerns and identify trends. Collaborates with involved teams and/or departments to review and address events.
  • Facilitates root cause analysis and identifies improvements related to patient safety and prevention of harm through the use of process improvement methodologies and tools.
  • Provides a report with deficiencies identified during internal surveillance and monitors the status of improvement implementations to key stakeholders.
  • Uses evidence-based patient safety practices to support, develop and implement initiatives (presentations, training programs, etc.) across the organization. Provides education and resources and identifies opportunities for improvement in patient safety compliance.
  • Leverages lessons learned and communicates recommendations and/or required actions to avoid future exposure of harm to patients. Collaborates with care management teams, clinical discipline teams, facility leadership and other groups as appropriate.
  • Provides support to internal and external customers to further the culture of safety.
  • Ensures security and privacy of data in all our systems.
  • Performs other duties as assigned.

MANDATORY MINIMUM QUALIFICATIONS

Experience: Three (3) years of direct experience in healthcare or patient safety related experience.

Education: Associate’s degree in Health Care Administration, Health Sciences or related field.
