Job Description
Overview:

The Intake Specialist is a part of the centralized intake and referral team for Amivie(Bloomignton, Mn) office. This position ensures effective and timely follow-up and communication with referral sources.



Responsibilities:
  • Receive, assess, and process referral inquiries. Assess clinical needs and supports to ensure service eligibility and insurance coverage. Ensure timely response to inquiries at provider level.
  • Act as a resource to individuals that are not eligible for service lines under CBC service array.
  • Enter and track referral inquiries into database and/or electronic health records. Report weekly/monthly referral trends as applicable to the Intake Director.
  • Support coordination into new services to include: collecting clinical documentation, supporting with rate approval/negotiation for family referred staff, submitting staffing requisition based on clinical need of referral.
  • Assist with scheduling of facility tours, assessment days and intakes as needed for providers.
  • Participate and present clinical documentation for review on weekly residential referral calls, to ensure referral meets ICF LOC. Support program/referral through residential referral process, working closely with program director and clinical supervisor.

  • Develop long-term relationships with referral professionals to establish ongoing dialogue about potential referrals and service system capacity.
  • Maintain current knowledge of the service system and resources available to persons served.
  • Provide excellent customer service to persons seeking services, their families, and professional referral sources.
  • Other duties as assigned.


Qualifications:

This is a full-time, hourly non-exempt, and due to the nature of the role, will require:

  • Bachelor's Degree in human services with two years of full-time, post-graduate experience with the population served; or Bachelor's Degree in non-human service field with four years of full-time, post-graduate experience with the population served; or Master's Degree in human services field with one year of full-time, post-graduate experience with the population served
  • Ability to work independently and as a part of a team and manage time effectively
  • Excellent interpersonal/communication skills.
  • Computer literacy, including proficiency with G Suite array of productivity tools (gmail, word processing and spreadsheet documents).

In Person postition at our Bloomington office.

  • Hours Monday-Friday 8am-5pm
