Description

We are currently seeking a full-time compassionate Family Support Coordinator for our Family Resource Center to assist families who have sons and daughters eligible for DDS services (Department of Developmental Services). This person will develop and implement a plan that assists the family in obtaining support and services.



They will provide home based services to families to assist in identifying their needs in a way as to translate their needs into services and community supports.



The Family Resource Center at Communitas, Inc. provides for families who have a family member with a developmental disability. We offer a wide variety of services including information and referral, case management, workshops, recreation and much, much more. We typically support Everett, Lynnfield, Malden, Medford, Melrose, North Reading, Reading, Saugus, Stoneham, and Wakefield.



Duties and overall responsibilities vary in different positions:



For Departmental of Developmental Services (DDS)




  • Inform the family and assists them in accessing supports and services that exist within the community.

  • Assists families in obtaining all state and federal entitlement benefits such as SSI,

  • CommonHealth clinical/medical supports, and behavioral supports.

  • Have an understanding of the Massachusetts Special Education Laws

  • Assists families in working with collaterals

  • Collaborate with other agencies

  • Supervise support staff working in families’ homes



For Department of Elementary and Secondary Education (DESE)




  • Assist families in coordinating together the various pieces of their intensive home programs. These pieces include working with Behavioral, Speech and Occupational Therapy

  • Consultants and staff to help individuals build skills.

  • Work collaboratively with school and other agencies

  • Completing necessary documentation for DDS

  • Managing budgets and processing expenditures

  • Supporting families through transitions



 

Qualifications



  • Bachelor Degree in social work, education, psychology or related field required.

  • At least two years of family support experience preferred.

  • Reliable transportation is required.

  • Clear CORI and suitable fingerprint results are required.
