The MICA Case Manager, plays a crucial role in supporting individuals as they navigate available services, helping them access life-changing resources and empowering them on their journey toward wellness and stability.
In this role you will:
- Assist residents with all paperwork relating to benefits and attend any re-certifications that are scheduled.
- Assist residents with discharge planning and with continuation of care after discharge from the MICA program
- Teach resident healthy independent living skills
- Act as an effective role model for the residents
- Provide supportive, psych-rehab goal oriented counseling when needed, with a focus on assessing and developing psych readiness.
- Familiarize residents with community resources for financial, medical, employment, educational, and recreational needs
Position Details:
- Case Manager MICA - Salary: $ 18.50 / Hour
- Schedule: Schedule: Monday – Friday 8:00AM – 4:00PM
Qualifications:
- Education:
- High School Diploma or GED
- Experience:
- 1–3 years’ experience with relevant experience in Human Services or a related field
- Skills:
- Microsoft Office
- Using databases
- Ability to maintain strict confidentiality
- Microsoft Office
- Other requirements:
- Valid Driver's License
- Proof of current car insurance and registration and access to a reliable, working vehicle
