As an RN Supervisor, you will play a pivotal role in ensuring the highest standards of patient care and supporting our team of caregivers across Albany, Saratoga Springs, Schenectady, and Rensselaer.

Key responsibilities include:

• Conducting in-home client assessments and supervisory visits to ensure quality care delivery
• Developing and reviewing individualized care plans for clients
• Providing case management oversight and maintaining quality assurance standards
• Supervising and supporting home health aides and caregivers in their daily responsibilities
• Maintaining accurate documentation and ensuring compliance with regulatory requirements
• Collaborating with the office team to coordinate care and resolve client needs
• Participating in occasional on-call shifts, with additional compensation
