What You Will Do
- Follow the individualized service plan (ISP) for each assigned resident
- Observe residents for changes in condition and report any concerns immediately to the nursing supervisor
- Document daily care, including observations and any changes throughout the shift, especially during rounds
- Check and record vital signs, weights, and perform other duties in accordance with state regulations and community policies
- Assist residents with activities of daily living (ADLs), including bathing, grooming, oral care, toileting, and dressing
- Perform restorative care techniques such as proper positioning, ADL support, and passive range of motion exercises
- Provide redirection, cues, and supportive communication to residents as needed
- Encourage and assist residents in participating in scheduled activities
- Assist with transfers and ambulation using proper body mechanics and equipment
- Provide transportation within the community and accompany residents to appointments as needed
- Respond promptly to call systems and resident needs
- Perform other duties as assigned by the supervisor
Why You Are Qualified
- High school diploma or equivalent (GED) required
- Nursing Assistant Training Program certification (as required by state regulations)
- Certified Nursing Assistant (CNA) certification preferred
- CPR and First Aid certification (if required)
- Ability to perform physical tasks including standing, walking, bending, lifting, and assisting residents throughout the shift
- Must be able to lift up to 50 pounds and assist with resident transfers using proper equipment
