The RN Case Manager will serve as primary contact person and coordinator for adults with disabilities in a group home setting in Berkshire County.

- Perform heath care assessments and develop written health care plans
- Consult with program staff on specific medical interventions, techniques and documentation requirements.

Requirements
- Current Registered Nurse license with at least one year experience. Some experience with people with developmental challenges preferred.
- Valid driver’s license & Insured Vehicle
