    Greenwich Public Schools seeks a Special Education Transition Services Facilitator.

    Position Purpose
    The Special Education Transition Services Facilitator, under the supervision of the Chief Officer of Special Education and Student Supports, is responsible for the coordination and facilitation of transition services, including transition assessment, transition planning, interagency linkages, and family support for students with disabilities ages 14–22. The Transition Services Facilitator is a key member of the Planning and Placement Team (PPT) and helps guide discussions to ensure the development of compliant, high-quality, and measurable postsecondary goals and transition services within the IEP. The Transition Services Facilitator serves as a liaison with adult service providers, colleges, training programs, and other postsecondary partners. In addition, the Transition Services Facilitator coordinates the District's high school and transition program work-based learning experiences for students by developing job sites, training job coaches, and placing students in community-based work settings. 
                                                                                                       
    Essential Responsibilities 
    • Provides direct instruction to students with disabilities to address individual transition needs, including pre-employment, community, independent living, and self-advocacy skills.
    • Develops curriculum, implements lesson plans, and assesses student progress in alignment with CSDE transition competencies.
    • Shares information with staff, students, and families regarding student needs, progress, resources, and transition timelines.
    • Coordinates transition activities that support college, career, and community readiness for students receiving special education services.
    • Plans and delivers parent/family seminars and staff professional development on transition services, Indicator 13 compliance, and community-based instruction.
    • Facilitates referrals, applications, and eligibility processes for adult service providers (e.g., DDS, BRS, DMHAS).
    • Develops and maintains collaborative partnerships with employers, community agencies, and postsecondary programs to support work-based learning and transition pathways.
    • Attends PPT meetings to support transition assessment, measurable post-school goals, and coordinated transition services.
    • Serves as the College Board SSD Coordinator and ACT TAA Coordinator, including processing accommodation requests and managing school-based SAT/ACT assessments.
    • Participates in transition-related professional development and CSDE training.
    • Serves as liaison between district transition programs (e.g., Community Connections) and community partners, employers, and postsecondary/training providers.
    • Collaborates with students, families, and IEP teams to develop and implement post-school goals related to employment, education/training, independent living, and community participation.
    • Monitors student progress in transition programs and coordinates internships, vocational and independent living instruction, job coaching, and related community experiences.
    • Ensures compliance with IDEA and CSDE guidance regarding measurable post-school outcomes and transition services in the IEP.
    • Provides training and support to staff, students, and families regarding transition resources, eligibility processes, employment services, and community supports.
    • Conducts outreach and partnership development with local businesses and agencies to support work-based learning and community-based programming.
    • Administers transition assessments at the high school and postsecondary levels.
    • Performs other duties as assigned that support effective transition service implementation.
    Knowledge, Skills and Abilities 
    • Demonstrated knowledge of state and federal laws and regulations pertaining to Transition Services for Students with Disabilities (IDEA 2004, Connecticut IEP standards, Indicator 13 requirements).
    • Ability to work effectively with students with varied needs, backgrounds, and transition pathways.
    • Knowledge of evidence-based transition practices, models of instruction, and community-based learning.
    • Excellent interpersonal, written, and verbal communication skills.
    • Ability to utilize learning management systems and technology to engage students and track progress.
    • Strong organizational, planning, and coordination skills.
    • Capacity for self-evaluation, reflective practice, and continuous professional growth.
    Qualifications Profile
    • CT certification, or certifiability, as special education teacher (e.g., #065), school social worker (#071), school counselor (#068), or in related certification area permitted by CSDE, required
    • Experience working with students with disabilities and implementing transition planning required
