Position Summary
Jewish Child Care Association (JCCA) is seeking a Family Nurse Practitioner (FNP) or Pediatric Nurse Practitioner (PNP) to provide comprehensive primary and preventive health care services to children and adolescents (ages 6–21) within JCCA’s Medical Program.
This role supports the delivery of equitable, high-quality, and trauma-informed care to youth in foster care, residential programs, and community-based settings. The Nurse Practitioner will work collaboratively with pediatricians, behavioral health providers, and interdisciplinary program teams to promote prevention, effective treatment, and long-term wellness for every young person served.
The Nurse Practitioner practices within an interdisciplinary team environment, and contributes to staff education, care coordination, and quality improvement efforts across JCCA’s medical services.
Schedule & Work Environment
- Full-time, 9:00 AM – 5:00 PM, weekdays (Monday–Friday), with some flexibility required
- Structured clinical setting with strong interdisciplinary support
- Active work environment with regular interaction with children, adolescents, and staff
Key Responsibilities
Direct Patient Care & Clinical Practice
- Provide direct primary care services to children and adolescents, including health assessments, diagnosis, treatment, and ongoing management of acute and chronic conditions
- Develop and implement individualized, developmentally appropriate plans of care focused on prevention, wellness, and continuity of care
- Prescribe medications and therapeutic interventions in accordance with clinical guidelines and regulatory standards
- Order, perform, interpret, and review diagnostic tests and procedures
- Deliver preventive health services, including health education and family planning counseling as appropriate
Care Coordination & Collaboration
- Collaborate closely with JCCA pediatricians and interdisciplinary clinical teams to ensure integrated, patient-centered care
- Coordinate referrals to medical specialists, community providers, and external clinics, supporting continuity of services
- Assist staff in navigating and maintaining partnerships with community-based health providers
- Serve as a clinical resource to program staff, supporting effective care delivery and client advocacy
Quality, Compliance & Education
- Ensure adherence to HIPAA, clinical standards, and agency policies
- Participate in quality assurance activities, clinical reviews, and documentation audits as needed
- Provide training and education to staff and clients related to health conditions, prevention, and wellness
- Support best practices in trauma-informed, culturally responsive care
Qualifications & Requirements
- Master’s Degree in Nursing required
- Current New York State Registered Nurse (RN) license in good standing
- Current national board certification as a Family Nurse Practitioner (FNP) or Pediatric Nurse Practitioner (PNP)
- Experience working in pediatric, adolescent, or family health settings strongly preferred
- Strong organizational, communication, and clinical decision-making skills
Ability to collaborate effectively with medical, clinical, and non-clinical staff
