Description

The Clinical Therapist provides culturally sensitive mental health services to the communities by providing assessments, referrals, and follow-up appointments that include developing plans with the client and implementing therapeutic treatment plans in a strength-based model in both the doctors’ offices and the individual counseling offices.   

 

This position is full-time on-site in Tustin, CA with a schedule of Monday through Friday 8am-5pm. 

 

The best candidate for this position:
  • has either an ASCW, AMFT, or APCC license or LSCW, LMFT, or LPCC license
  • has experience mobilizing community resources, navigating through the healthcare continuum, and working with disparate populations. 
  • understands diagnosis and treatment of mental health problems commonly experienced by individuals experiencing homelessness and the uninsured.

 

What You'll Do: 

Job Responsibilities & Duties

  • Provides direct behavioral health clinical therapeutic and integrated medical services to individuals, families, groups and communities, under the supervision of the Behavioral Health Manager.
  • Assess client’s strengths, needs, abilities, and preferences and formulate a diagnostic impression based on current DSM criteria. Perform biopsychosocial assessments and develop collaborative treatment plans for all ongoing clients.
  • Discusses progress toward goals with client such as controlling weight, controlling pain, adjusting to stress, or substance abuse education and support services.
  • Determine therapeutic approach based off of client’s needs (i.e. treatment frequency and duration of therapy).
  • Provides crisis intervention counseling as needed and outreach counseling to community settings such as schools, home visits, and other settings as assigned by the Behavioral Health Manager.
  • Provides specialized counseling groups or classes for adjustment to illness, adjustment to pain, parenting classes, depression, grief, anger management, teen issues or on other topics within the scope of license and training.
  • Maintains accurate and timely clinical documentation consistent with agency standards.  Documents progress/case notes in the EMR that include diagnostic mental health assessments, treatment plans, and progress notes in addition to other reporting and statistical data in a timely manner as required by organizatinal and legal guidelines.
  • Conducts relaxation exercises learned from supervision to facilitate peer counseling groups and family counseling during clinical therapy sessions.
  • Consult with relevant care team members through case discussions and presentation of client’s family and social background pertinent to diagnosis and treatment.
  • Works cooperatively with outside community resources and other in-house services (i.e. medical, dental, etc) or support programs to supplement in office treatment and counseling to coordinate services for clients.
  • Adheres to accreditation and compliance standards/guidelines and gets yearly culturally supportive training.
  • Assists in development of departmental policy, procedures, and quality improvement activities within the clinic as directed.
  • Periodically provides community mental health presentations and PowerPoint presentations on behavioral health preventative measures that includes but is not limited to pain management, childhood mental health issues, or stress management workshops to the community and/or clinic staff.
  • Inputs and maintains accurate patient information in the EMR system in a timely manner.
  • Works to meet client needs effectively and resolve individual barriers through patient follow up. Scans all paperwork pertaining to patients into the EMR.
  • Performs any and all such duties that are within the scope of this individual’s certifications that are requested by the medical providers.
  • Develops community relationships for patient referrals.
  • Tracks and maintains all logs for patient referrals.
  • Makes sure patient and provider have signed all appropriate paperwork when applicable.
  • Works closely and collaborates with other Behavioral Health and Hurtt Family Health Clinic staff to provide the best possible care within scope of practice and services.

  • Education & License: Master's degree & ASCW, AMFT, or APCC license (Level II)* or Master's degree & LCSW, LMFT, or LPCC license (Level III)*
  •  NPI number, or obtain an NPI prior to employment start date
  • Education: Registered in California as an Associate Clinical Social Worker (ACSW), Associate Marriage & Family Therapist (AMFT) or Associate Professional Clinical Counselor (APCC).
  • Two years (3-5 years preferred) of clinical experience and preferably some experience in a medical/clinic setting. Previous work in a Community Clinic (FQHC) setting is preferred.
  • Current Basic Cardiac Life Support (BCLS) certification for infants and adults.
  • Must be familiar with providing specialized counseling groups or classes for adjustment to illness, adjustment to pain, parenting classes, depression, grief, anger management, teen issues or on other topics within the scope of license and training.
  • Comprehensive knowledge of community resources, and public and private benefits programs.
  • Familiar with various funding sources such as Medi-Cal, Medi-Care, etc.
  • Electronic Medical Records/Electronic Health Records Data experience is preferred but not required.
  • Ability to work effectively in a multicultural environment with diverse groups of men, women, and children.

  • Ability to recognize and maintain confidentiality of information as appropriate.
