Description

The Nurse Practitioner provides primary care services of an acute, chronic and preventive nature to patients of all ages groups as guided by organizational protocols and license to practice. 

Requirements

  • Current state licensure as a Nurse Practitioner is required.
  • Graduate of an approved Master's level or Doctorate's Level course of Nursing Practitioner education is required.
  • Certification by a National Nursing Organization and licensure by Boards of Nursing and Medicine is required.
  • Two years' work experience preferred.
  • Must maintain certification as Basic Life Support Provider.
  • Must maintain current state medical license and DEA certificate and provide to Human Resources.
  • Must maintain updated copies of all licenses, certifications and CME.
  • Ability to work in a cooperative fashion with diverse groups of health professionals and center staff.
  • Ability to work independently and make appropriate decisions which reflect professional strength, confidence and integrity.
  • Must have excellent oral, written and interpersonal communication skills.
  • Must have basic keyboarding skills and computer knowledge.
  • Must demonstrate excellent internal and external customer service skills.
  • Employee must demonstrate an understanding of the intent, and follow standards, guidelines and protocols related to achieving and maintaining certifications by outside agencies.
  • Must demonstrate a personal and professional commitment to Eastern Shore Rural Health and its Mission Statement.

Summary

  • Will provide primary care services of an acute, chronic and preventive nature to patients of all age groups as guided by organizational protocols and license to practice.
  • Will function as a interdependent member of the health care team with both dependent and independent functions.
  • Adheres to quality assessment guidelines, OSHA or other regulations as federally mandated or as set forth by principles of practice of ESRHS.
  • As part of the center team, consisting of other providers, nurses, health education specialists, case managers, interpreters, transportation and outreach workers and others, assists in implementation of the clinical services plan with measurable goals and objectives as outlined in the Health Care Plan of the Federal 330 Grant.
  • Accepts clinical responsibilities/duties assigned or requested by the Chief Medical Officer or Chief Executive Officer.
  • Assists in the responsibility of call for the practice.
  • Abides by policies, procedures and protocols as needed to achieve the center's goals and objectives.
  • Represents ESRHS in the community through involvement in community activities which support the mission and philosophy of ESRHS.
  • Maintains organized, complete electronic medical records with regard to content appropriateness and confidentiality.
  • Participates in center staff meetings, sharing responsibility for effectiveness of these meetings when requested with center manager and clinical director. Supports and assists the clinical staff as needed, makes recommendations for service improvement, patient flow and changes to center.
  • Shares in responsibility of teaching of students, residents, and other health professionals as the community health center becomes involved in the training of primary care providers.
  • Attends quarterly ESRHS provider meetings unless prevented by unusual need of center functions or hospital call duties. If unable to attend, must notify CMO for approval. Every other quarterly provider meeting may be viewed at the medical provider's center.
  • Assists in weekend provider coverage at ESRHS Express Care as needed, or seasonally on Saturdays at Chincoteague Island Community Health Center.
