RN Case Manager – Hospice

Job Type: Full-Time | Part-Time | PRN Available
Reports To: Manager, Clinical Operations
Location: Arlington, TX + Local Territory

What You’ll Do

As an RN Case Manager, you will:

  • Provide expert nursing care for home and facility-based hospice patients
  • Serve as the primary clinical and communication lead for your patients
  • Coordinate care with families, physicians, facility staff, and the interdisciplinary team
  • Build trust quickly through empathy, education, and clear, thoughtful communication
  • Assess, anticipate, and respond to changing patient needs
  • Identify and address issues before they escalate
  • Navigate emotionally complex situations with professionalism, sensitivity, and confidence
  • Support families and caregivers with education, reassurance, and meaningful presence
  • Advocate for patient dignity and deliver comfort through both clinical care and human connection

What You’ll Need

  • Current Texas RN License
  • Valid Texas driver’s license, reliable transportation, and auto insurance
  • Graduate of an accredited Diploma, Associate, or Baccalaureate nursing program
  • 1+ years of nursing experience (hospice, palliative care, oncology, home health, or acute care preferred)
  • Ability to work Monday–Friday (8:30 AM – 5:00 PM) with rotating weekend on-call availability (full-time)
  • Strong time management, organization, and documentation skills
  • Experience with EMR systems (Consolo EMR a plus)

Compensation & Structure

Full-Time:

  • Starting salary: $82,500 – $105,000+ (based on experience)
  • Auto Allowance
  • Phone Allowance
  • Health Insurance Benefit Allowance
  • Performance-based incentives tied to quality, communication, and outcomes

PRN / Part-Time:

  • Competitive per-visit rates ($75 – $95 routine; higher for admissions/recerts)
  • Flexible scheduling based on availability and geography

Preferred (Not Required)

  • Bilingual (Spanish/English)
  • BSN or higher
  • Experience in hospice, home health, or case management
  • Familiarity with Consolo EMR
