Licensed Practical Nurse (LPN) – Day Shift
$5,000 Sign-On Bonus
Avondale Health and Rehabilitation Center – Humboldt, TN
Full-Time or PRN | 6:30 AM – 6:30 PM | Skilled Nursing / Long-Term Care

Avondale Health and Rehabilitation Center is seeking compassionate and dependable Licensed Practical Nurses (LPNs) to join our growing team in Humboldt, Tennessee. In this role, you will provide direct resident care, administer medications, and support our clinical team in delivering high-quality care in a skilled nursing and rehabilitation facility.

We are currently hiring for Full-Time and PRN Day Shift (6:30 AM – 6:30 PM).

Pay Rates

$22.00 – $28.00 per hour
$24.20 – $30.80 per hour → MaxPay Program (ask during interview for details)

$5,000 Sign-On Bonus available for qualified candidates


LPN Requirements

  • Active Licensed Practical Nurse (LPN) license in Tennessee
  • Experience in long-term care, skilled nursing, or rehab preferred
  • Strong clinical and assessment skills
  • Excellent communication and teamwork abilities
  • Ability to prioritize and work in a fast-paced environment
  • Willingness to work weekends and holidays as needed

LPN Responsibilities

  • Provide direct nursing care according to individualized care plans
  • Administer medications and treatments as ordered
  • Monitor resident condition including vital signs and changes in status
  • Complete accurate and timely documentation
  • Assist with admissions, transfers, and discharges
  • Work closely with RNs, CNAs, physicians, and therapy staff
  • Support residents with daily care needs when needed
  • Participate in care planning and family meetings
  • Follow infection control and safety procedures
  • Help train and support new nursing staff
