Job Description

IMMEDIATE SUPERVISOR: This position will report to the Addiction Services Program Coordinator.

GENERAL FUNCTION: The Alliance Healthcare Services Enhanced Outpatient Program (EOP) provides comprehensive team-based services for clients with substance use and/or co-occurring disorders. The EOP Care Coordinator will be responsible for providing care coordination services for clients on their caseload.

SUPERVISORY RESPONSIBILITY: None.

QUALIFICATIONS; KNOWLEDGE AND REQUIRED TRAINING: Bachelor’s degree in Psychology, Human Services or related field from an approved university training program required. Must be eligible for any/all credentialing required by regulatory and funding sources. Previous experience in working in a community mental health setting is preferred. This individual will be part of a multi-disciplinary treatment team and will require collaboration and consultation with team members of various roles as well as ability to maintain positive relationships with community stakeholders and must demonstrate an ability to use mature judgment in problem solving.

DUTIES AND RESPONSIBILITIES:

• Provide care coordination services in an outpatient and community setting.

• Complete DLA-20 and other functional assessments on clients who agree to or currently receive services.

• Complete Treatment Plans for clients on caseload or at intake.

• Assist with intakes as assigned or requested.

• Understand and monitor quality metric/HEDIS outcomes for clients served.

• Provide resources and referrals and help clients grow independence by teaching them how to access these on their own.

• Complete both THL and BHSN service through health promotion, transitional care services, providing member and family support, referring to social supports, and coordination with other providers.

• Demonstrate a trauma-informed, person-centered approach to treatment.

• Participate in regular clinical supervision and treatment team meetings and provide clinical input on all clients in the program.

• Involve family members and natural supports as available/appropriate in treatment process.

• Maintain positive and collaborative community and peer relationships to promote the mission of Alliance.

• Maintain appropriate boundaries with clients, and alert supervisor if a conflict of interest arises due to knowing a client from the recovery community outside of Alliance.

• Consistently achieve agency required productivity goals.

• Complete high quality, collaborative documentation on all visits provided.

• Prioritize Opioid Abatement Grant goals and required documentation.

• Adhere to Alliance standards of professionalism by being punctual and dependable, neat and professional in appearance, maintaining an enthusiastic approach to completing assignments, standing out as a positive role model for others, utilizing appropriate methods to cope with stress, accepting constructive criticism and integrating suggestions in effective ways, and working with others to promote cooperation to accomplish the agency mission.

PERSONAL RELATIONSHIPS: In order to appropriately accomplish his/her duties, the person occupying this position will be required to maintain effective relationships with the professional clinical staff and the clients of Alliance Healthcare Services.
